At the beginning of the book, the author presents a talented couple. Gabriel Berenson is a famous photographer, while his wife, Alicia, is a painter. Married for seven years, one day in August, Alicia shoots her husband many times in the face. When their neighbor, Barbie Hellman, calls the police, Alicia looks stunned into immobility and with slit wrists.
The conflict of the book arises when Alicia murders her husband. Although Alicia becomes quiet, she creates her most scandalous artistic work, the Alcestis. She instantly receives condemnation from the public for returning to painting soon after the death of her husband.
With Alicia’s trial and self-portrait gaining immense popularity, Jean-Felix, Alicia’s gallerist, decides to showcase it in an exhibition. Due to media criticism, she is branded a ruthless killer.
During the litigations, Alicia remains mute, and her defense enters a plea of diminished responsibility, presenting her history of mental illness. Eventually, a forensic psychotherapist, Lazarus Diomedes, sways the unyielding judge by confirming that Alicia formerly suffered mental health issues. Following Diomedes’s professional opinion, the court accepts the plea of diminished responsibility, and Alicia gets admitted to the Grove under Diomedes’ supervision.
I love the artful nature of Alicia’s silence after her alleged murder. I think as an artist, she did it for the symbolism.
Theo Faber, another protagonist, is a 42-year-old man who becomes a forensic psychotherapist because he suffered a psychotic disorder from an abusive childhood.
An unpredictable hothead, Theo’s father physically abused him whenever he was triggered. When Theo leaves for university, he realizes he is significantly damaged. Hearing voices in his head and unable to socialize, he attempts suicide but survives.
Afterward, he seeks help and meets a psychotherapist, Ruth. After numerous therapy sessions, Theo feels more peaceful and chooses psychotherapy as a vocation.
Since a therapeutic intervention transformed his life, Theo plans to use therapy to restore Alicia to sound health. He obtains a job at the psychiatric hospital she belongs to.
I believe Theo’s earlier experiences with his father shaped his mental health. I also think his submission to therapy was a genuine cry for help as he wasn’t idly waiting for intervention from anywhere.
On the first day at the Grove, Theo joins a group therapy session that includes patients and staff. As he studies the patients, he finds Alicia, who has lost weight, and her attractive features. Heavily medicated, she has no idea of what is happening around her. Observing Alicia’s state of disorientation, he vows to treat her.
Following his resolve, he requests therapy sessions with Alicia from Diomedes. Although Diomedes is against it, Theo insists. Diomedes eventually agrees, and he begins his sessions. Alicia attends the first therapy session with Theo but remains mute, forcing him to sit silently.
To learn more, Theo collects a file containing information about Alicia. He makes notes on ideas and plans needed to help her. He gets cold and rude responses when he reaches out to Alicia’s aunt and brother-in-law. Indulging a conversation with Alicia’s nurse, Yuri, Theo hopes to get more information; instead, he gets advised to leave Alicia alone and return to his wife. Even with this warning, Theo becomes obsessed.
Theo’s romantic relationship with Kathy is joyful. They meet while dating other people, fall in love, and get married. At the end of this chapter, Theo compares Kathy and Alicia, stating that while Kathy reminds him of warmth and light, Alicia’s silence reminds him of darkness and sadness.
I believe that Theo wants to save Alicia, he also insists on helping her because it is a push in the right direction career-wise.
The second part of the book presents Alicia’s diary notes before the death of Gabriel. She describes how she longs for rain and spends time in open and air-conditioned places to deal with the scorching heat. At home, Alicia buys a fan that Gabriel complains about because of the noise it makes. Alicia takes the fan everywhere because the heat interferes with her work progress.
She paints a picture of Jesus on the cross, but when she looks closely, the face and body are Gabriel’s. Alicia thinks about asking him to pose for the portrait since she unconsciously painted Gabriel.
While visiting Camden market one morning, Alicia runs into a homeless man. She thinks about her mother, who died by driving her yellow car through a brick wall with Alicia strapped in the passenger’s seat. Alicia becomes distressed, contemplating the sanity of her mother at the time.
In her diary, Alicia explains the affection that exists between her and Gabriel. As a romantic and supportive partner, he makes Alicia feel loved. After their date, Alicia asks Gabriel to pose, so she paints him. Skeptical at first, he finally agrees to the excitement of his wife. Alicia confesses to love Gabriel profoundly and will forever do so.
I think Alicia is a very emotional person, contrary to the thoughts of the critics.
The story switches back to the perspective of Theo at the Grove. He requests the reduction of Alicia’s medication dosage so that she can think and feel. Diomedes is not receptive to the idea because Alicia is liable to violent eruptions. A few days after the dosage reduction, Alicia attacks Theo during their session. Theo sees her assault as a form of communication. The Grove’s supervisor, Stephanie Clarke, asks that Alicia’s therapy stop, but Diomedes refuses.
At home, Theo secretly smokes marijuana since his wife is absent. While stoned, he stumbles on emails on her laptop that prove she is cheating on him and becomes devastated.
During the next therapy session, Theo uses the anger and pain at Kathy’s betrayal to connect with Alicia. He states that he sometimes hates his wife even though he loves her, so Alicia had a part of her that hated Gabriel. After shaking her head to imply “no” twice, Alicia angrily walks out of the session.
I think Theo is good at his job. Accepting Alicia’s attack as some communication is something I expect from a professional therapist.
Theo faces mental distress as his perception of Kathy takes a rollercoaster. He decides she is a liar and questions her love for him. Eventually, Theo goes to his university therapist, Ruth. She listens to him, and when he says Kathy is cheating because she’s bored, she reminds him that real love is quiet and boring. When Theo tries to make more excuses for Kathy’s infidelity, Ruth tells him there is a childhood dynamic replaying.
Although Ruth tells him he deserves someone better than Kathy and should leave her, he does not confront her when he returns home. Kathy accuses him of being secretive when she finds his jar of weed. Ignoring her accusations, Theo goes to bed and cries silently. The following day, he ignores his wife’s infidelity and tries to move on.
While talking to Elif, later, he gets a call from Alicia’s brother-in-law and lawyer, Max Berenson, who agrees to meet with him. Max confesses he refused to see Theo earlier because he thought he was a journalist. He reveals that he hated Alicia because she destroyed Gabriel’s life after threatening to kill him several times. He also states that Alicia attempted suicide after the death of her father. Max’s secretary discreetly advises that Theo talk to Alicia’s cousin, Paul.
In Alicia’s diary entries, she illustrates her arguments with Gabriel over the gun he keeps in the house. She confesses that while she fears having a child and pretends not to want one, she does. She also states how wary of Max she is. When Gabriel invites Max for dinner, Alicia dodges him, but he finds her, and Alicia threatens to tell Gabriel he sexually assaulted her at a party. As Alicia confronts him, Max grabs her and forcefully kisses her. She struggles and bites his tongue. Afterward, he warns her against confessing to his brother.
To me, Theo does not confront his cheating partner so he does not lose her. He fears her abandonment more than he hates her infidelity.
During her 33rd birthday, Gabriel gives Alicia thirty-three flowers and takes her to a park picnic. Gabriel asks her to have a child, and she agrees happily.
When Theo asks Max about Gabriel’s Will, implying that Max was involved in Alicia’s mishap because he wanted to inherit Gabriel’s estate, he gets reported to Diomedes. Diomedes cautions him against riling up a lawyer who could bring the Grove lousy publicity.
Although he promises not to speak to any of Alicia’s relations, Theo’s obsession leads him to visit Paul, Alicia’s cousin. After talking to both Paul and his mother, Lydia, he gets a picture of the toxic environment Alicia grew up in. Paul believes Alicia is innocent because she is unable to harm anyone. On the other hand, Lydia is hostile and angry with Alicia for painting her in a monstrous image.
I believe Theo digging into Alicia’s past makes him understand her personality better.
Although Theo does not confront Kathy about her infidelity, he feels insecure and is constantly on the lookout. When she asks if he is alright, Theo notes how good she is at pretending to care. She tells him that in a few days, she will visit her friend; Theo gets suspicious and decides to follow her.
Theo visits Alicia’s gallery and meets Jean-Felix. He is happy to talk about Alicia and takes Theo to the storage room where Alicia’s works dwell. When Theo sees Alicia’s famous Alcestis painting, he buys a copy of the book by Euripides. He is astounded when he realizes that in the book, Alcestis also remained quiet after she sacrificed her life for her husband and returned to life.
In Alicia’s diary, she speaks about her meeting with Paul and how he asks for money to clear the debt he incurred from gambling. She feels guilty about the amount she gives him to offset the debt. She also narrates how Jean-Felix walks into the house unannounced, and when she gets annoyed, he tries to guilt-trip her. When Alicia says she wants a change of gallery, Jean-Felix blames Gabriel for her decision and refuses to see reason.
I do not expect Jean-Felix to be a person Alicia likes as a friend. His lack of boundaries is a behavior that will keep her continually tensed.
Later on, Theo suggests to Diomedes that Alicia be allowed to paint. He gets approval and gets Alicia’s art materials from Jean-Felix.
Back home, Theo follows Kathy hoping to run into her secret lover, but she truly meets up with her friend. Theo is disappointed.
Alicia’s countenance improves when she begins to paint. Ultimately, she creates a picture of the Grove on fire and Theo helping her escape. Her progress is, however, interrupted when Elif destroys her work and Alicia stabs her eye with a paintbrush. This act earns her solitary confinement. Diomedes also end Alicia’s therapy sessions with Theo. When he informs her about the termination, Theo gets Alicia’s diary. The discovery of Alicia’s diary is a rising action in “The Silent Patient.”
I love how rewarding Theo’s efforts are. He would work better with Alicia when he reads her thoughts, fears, and motivations.
The turning point is presented as Alicia’s diary entries reveal her thoughts and actions to Theo. She sees a man watching their house from the other side of the road, behind a tree. Frightened, she leaves the window to paint. When she sees the man on another occasion, she telephones Gabriel, but he is busy.
When Gabriel returns home, Alicia tells him. At first, he shows concern, then suspects she is imagining events. Her neighbor, Barbie, suggests she informs her husband when she’s told about the man. After Alicia’s failed attempt to show Gabriel anything unusual, he worries about her mental state. He thinks she imagines the man and insists that she sees Doctor West. At Doctor West’s house, Alicia talks about being stalked, and he reminds her that they have gone through similar events. When West threatens the loss of Gabriel if she is not cooperative, she succumbs.
Soon, Alicia starts displaying symptoms of paranoia. She hides her diary as she trusts nobody. Alicia also thinks about Gabriel’s gun and decides to keep it close. Gabriel convinces her to go to dinner with Max and Tanya. At the restaurant, Alicia accuses Max of stalking her, and he denies it. At the end of this section, Alicia hears someone breaking into the house. She calls Gabriel, but he does not pick up.
I know Gabriel is unfamiliar with mental health challenges because of how he treats Alicia. Insisting a mentally disturbed person is paranoid increases their paranoia. They eventually stop telling you about their fears.
Theo tries to make sense of Alicia’s diary after reading it. Deciding that there’s more to Alicia than he thought, many questions form in his mind, including the identity of the man who was stalking Alicia. He confronts Doctor West, who is his colleague, Christian. Theo realizes Christian has been seeing other patients informally and did not declare his income. He promises to keep his secret if Christian tells him about his therapy sessions with Alicia.
Theo organizes a private session with Alicia when he leaves Christian’s office. When they meet, Theo asks why her diary ends abruptly. He says he expects her to open up and talk to him now that they are closer. However, Alicia remains silent throughout the session.
Theo follows Kathy and catches her sexual involvement with her lover. Burning with hatred, he wonders if this is God’s punishment for his mistakes. Theo contemplates killing the man as he and Kathy leave the woods.
I believe Theo’s mental illness fully returns when he sees Kathy with her lover. Expectedly, he wants to protect Kathy, but eliminate the person who tries to take her from him.
Deciding to provoke the characters in Alicia’s diary to unravel the truth, Theo begins with Jean-Felix. While waiting for him, Theo looks at Alicia’s Alcestis painting and discovers the dark part shows a prying man. Stubbornly refusing to admit Alicia was planning to leave his gallery, Jean-Felix becomes sober when Theo tells him he got the information from Alicia’s diary.
Next, he calls Max and confronts him about his romantic feelings for Alicia. Max hangs up. He also confronts Paul about his gambling. Paul again lies that although he asked for money to settle his debts, Alicia did not give him.
Paul explains how Alicia’s father killed her spirit by wishing death on her instead of his wife. With this information, Theo understands why Alicia becomes drawn to Alcestis.
Back at the Grove, Theo empathizes with Alicia. He also warns that this is her last chance to communicate. To Theo’s amazement, Alicia speaks. Mad with joy, Theo informs Diomedes, and Alicia’s therapy continues.
I think there is a switch from being a therapist to being a detective here. Theo probes his suspects with their hidden agendas and secrets.
Alicia tells Theo she refused to talk immediately after Gabriel’s death because she could not. She opened her mouth, but no words came forth. When Alicia found her voice, it was too late to say anything. She credits her speech restoration to Theo and tells him that the man who broke into her house in the last diary entry was Jean-Felix, and he wanted to talk about an exhibition. Alicia and Theo talk about Alicia’s childhood and then Theo’s childhood.
Soon, Alicia talks about the night Gabriel died. She says while trying to paint, she sees the stranger behind her. He attacks her, and the cabinet is empty when she tries to get Gabriel’s gun to kill him. The masked man ties her up and waits till Gabriel’s arrival. He hits Gabriel’s head with the butt of the gun and shoots him six times before leaving.
Theo does not believe her story because of the inconsistencies with the police report. Gabriel got shot five times and not six. Also, she was not tied up when the police arrived. Diomedes also confirms her narration is false. He disbelieves any man stalked Alicia. He advises Theo to challenge Alicia during the next therapy session and insists she has to tell the truth.
Again, Theo follows Kathy and finds her with her lover. Theo follows the lover and, unable to kill him, watches him enter his house and kiss a woman. Theo realizes the man is also cheating on his wife.
To me, Diomedes advises Theo to confront Alicia because they have become close enough for him to do that. Nobody confronted Alicia when she was mute and heavily sedated.
Confrontation is impossible as Alicia gets admitted to the ward the next day. She allegedly tries to commit suicide by overdosing on pills. Theo blames himself for this. When everyone leaves the ward, Theo discovers a bruise and mark suggesting someone had injected her with morphine.
Theo suspects Christian and tells Diomedes and Stephanie about his relations with Alicia before her admission to the Grove. He insists Christian had tried to murder Alicia to conceal the truth. Diomedes calls the police.
Theo returns to the house of Kathy’s lover. He walks into the house and wears a mask and black gloves. When he enters, he finds the wife painting a portrait. She catches sight of his reflection in the window and stiffens in fear. When she slowly turns around, she is Alicia Berenson. The revelation of Alicia and Theo’s meeting outside the Grove brings climax.
Alicia’s confirmation that she was getting stalked would have almost scared her to death. I believe that was one of her most helpless situations.
Alicia’s diary explains her true confession. Admitting she has little time left, she expresses how shocked she was to see Gabriel’s murderer had returned as a therapist. On the day of Gabriel’s death, Theo forces Gabriel to choose between his life and that of his wife after tying them up.
Gabriel chooses to save his life, bringing Alicia to her childhood trauma. She remembers her father wished her dead instead of his wife. Theo shoots the ceiling and leaves them alive. Traumatized afresh, Alicia vengefully shoots her husband five times.
Alicia lies to Theo about how Gabriel died to prove she remembers him. To silence Alicia permanently, Theo injects her with morphine, and she goes into a coma. This is the falling action as Theo tries desperately to make her return to muteness after the discovery.
While clearing out Alicia’s things, Theo looks for Alicia’s diary he had returned to her, but it is nowhere in sight. Soon after, He learns of Diomedes’s resignation and his recommendation of him for his position. He is elated.
Theo’s inexperience with crime is apparent to me. A professional murderer would have found and destroyed Alicia’s diary.
In the end, the cold relationship between Kathy and Theo is revealed. Kathy is depressed and refuses to talk to a therapist or go to work. Theo realizes he has gone a long way to keep Kathy, yet he lost her.
The resolution of the book occurs when a Chief Inspector visits Theo. After pleasantries, he informs him that Jean-Felix came to the Grove to collect Alicia’s art materials and found Alicia’s diary, where she unveils Theo as the man who injected her with a massive dose of morphine.
